Default How to upload to the internet?

Hi, I have a database form set up for people to input data on their symptoms, etc for the parkinson's forum. How do I get this form onto the internet. I have already formatted alot of the fields with dropdown boxes and made it "limit to list" to ensure that data is entered like I want it. Thanks.

By the way, I did the form in Access.

Unfortunately (or fortunately, depending on how you look at it), there's no easy way to make something you do on your computer available on the web. Access is not a database supported by any web host. You might consider, instead, using something like Google Apps, as you can put forms tied into their spreadsheet in order to gather data.

The real question, though, is what's the purpose of gathering this information? We don't allow any kind of research on NT.

We have a long time member who put up a spreadsheet format
for testing for PN.

She did this a long time ago. It is designed for posters here,
to use as a diagnostic aid when visiting their own doctor.
This helps the doctor see trends and avoid duplicate testing and also to perhaps test using less commonly known criteria.

The protocols for testing to find a cause of PN are complex and not all doctors know them.

This is a site she had someone make for her, so she could share the testing sheets, to enable each person to track their own
testing at their doctor(s). This poster does not keep any data, and only offers the platform testing sheets to be used privately.
