Your symptoms do not sound like parkinsons. The amount of levodopa you are taking is very low and 3 pills a day say morning, noon and night is too low to stay at an effective level in your body.
Do not mess around with anyone else's opinions here or elsewhere. Get yourself to a teaching hospital or a Parkinson's Center for Excellence. Dr. Okun is at Gainesville, UF. There is one in Tampa and Miami. I don't know where you live but it is worth the trip to get to one.
You will find out if it is PD or what it may be for sure. As far as your BP problems, those can be caused by the bad kidney. That is a primary cause of uncontrolled BP resistant to medications. My mom had a bad kidney and after surgery her BP is now normal.
Most of all get more of a specialist involved and then find a new neurologist if they have other things to do but address this issue.
Remember don't go to the appointments alone.

Default meds

the fact other meds (ie your BP meds) were increased/added along with sinemet is troublesome. what were the other meds? Any research into possible interactions between one or more of the BP drugs and sinemet? I well know the incredible, scary hallucinations associated with sinemet therapy, especially when other meds are added.
I recognize from your name that you do see an MDS at a major teaching hospital.
