Default reporting adverse experiences to FDA

As a consumer/patient, you can report an adverse experience that you have directly to FDA. You don't have to rely on your physician.

For approved/marketed drugs: http://www.fda.gov/medwatch/index.html This uses a simple form that you can fill in and send electronically.

For investigational drugs: http://www.fda.gov/oc/gcp/. This site gives you phone numbers that you can use.

In either event, your report will be directed to the appropriate FDA review division.

Here is an amazing website from Barcelona, Spain.
